Afterschool program registration form

In order to successfully register for afterschool parents must do the following: 

1 - Fill out the Afterschool registration Application. 

2 - Email the COMPLETED form to - enrollment@program-believe.com / on that email please state your name, phone number and your child's name and grade that he will be attending this upcoming year. 

3 - If a spot Is available for your child one of our administrative assistants will contact you with more information and instructions .
